Telehealth Consultation and Subscriptions
Telehealth Comprehensive Diabetes Care Consultation and/or Medication Review
A 40 minute session for: diabetes education, medication management, diabetes tech support or diabetes medication refills available to Washington residents.
Comprehensive Diabetes Care and/or Weight Management 6 month Subscription- Portal Messaging & Telehealth Visits
Six-month program: 15 minute phone, messaging or virtual appointments weekly or longer monthly appointments. . Available to Washington residents.
Comprehensive Diabetes Care and/or Weight Management Annual Subscription- Portal Messaging & Telehealth
1 year plan for flexible, ongoing support. 15 minute phone, messaging or virtual appointments weekly or longer monthly appointments. Available to Washington residents.
Weight Management Consultation
Consultation for Weight Loss Prescription. We provide the consultation and issue a 90-day prescription to your preferred pharmacy.
You pay the pharmacy for the medication separately. Available to Washington residents.
Comprehensive Medication Review
Our Medication Therapy Management (MTM) service provides a thorough review of your medications, including interactions and side effects. We guide you on optimizing your medication regimen for better health and effective management.
Senior Medication Therapy Support
For seniors facing polypharmacy, our expertise ensures safe, effective medication management. As a dedicated advocate and educator, she provides patients with clear, engaging educational materials tailored to diverse learning styles, helping them actively participate in their collaborative healthcare agreement.
Diabetes Self-Management and Wellness
Through our Diabetes Self-Management Training (DSMT), we offer personalized diabetes education to help you manage your condition and devices confidently. Our Diabetes Wellness program takes a holistic approach, addressing cardiometabolic health, weight management, and mental wellness to improve overall well-being.
Weight Management/Weight Loss
